Submitting this form will be the RSVP for each person listed. If no one is representing your church, please enter “none” in the first blank and send in the form anyway.

According to the CPBA Constitution and By-Laws:

A. Each church shall be entitled to five (5) messengers, plus one (1) messenger for each additional twenty-five members, or fraction thereof, above one hundred, provided that the total number of messengers from any one church shall not exceed nine (9), including the pastor. Messengers may be required to furnish evidence of having been properly elected by the churches.
